Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice.  This courtesy, on your part, will make it possible to give your appointment to another patient.  

Emergencies/After Hours

Our office provides on-call service to our patients for emergencies, 24 hours-a-day, 7 days-a-week by calling (541) 267-4429.

Prescription Refills Policy

1. The nurse will manage all prescription refill requests.
2. Please notify your pharmacy to place the refill request. No requests will be taken over the phone.
3. Please allow 2 business days for your request to be authorized/denied.
4. All pharmacies will fax prescription refill requests to our office.
5. The nurse will notify the doctor of the request and get authorization for the refill.
6. If the doctor authorizes the refill, the nurse will fax the prescription refill request form back to the pharmacy for refill
